Breast implants, technically known as ‘enlargement mammoplasty‘, is a surgical procedure to enhance the size and shape of a woman’s breast for a number of reasons:

  • To enhance the body contour of a woman who, for personal reasons, feels her breast size is too small
  • To correct a reduction in breast volume after pregnancy
  • To balance a difference in breast size
  • As a reconstructive technique following breast surgery
  • To enhance contour of breasts after significant weight-loss

The breast consists of milk ducts and glands which are surrounded by fatty tissue, and covered by skin; the fatty tissue is what gives the breast its’ soft feel and shape.

Skin elasticity also affects breast shape. Pregnancy (during which the milk glands are temporarily enlarged) combined with the inevitable effects of gravity over time can stretch the skin envelope and change breast shape. The pectoralis major muscle lies beneath the breast, and indirectly affects breast projection. The pectoralis major muscle helps with flexing and rotation of the arm and shoulder joint. As well as breast enlargement, there are also procedures for breast reduction if you feel your breasts are too large.

Who is the best candidate for a Breast Augmentation procedure?

The best candidates for breast enlargement are women who are looking for improvement, not perfection, in the way they look. If you’re physically healthy and realistic in your expectations, you may be a good candidate.

The Procedure

In most cases, breast enhancement surgery can be completed during one day under local anesthetic with sedation, or as an overnight treatment with general anesthetic.

As well as breast enlargement, there are also procedures for breast reduction if you feel your breasts are too large or are having health implications due to their size. In both of these types of surgery, small incisions are made to either remove some of the breast tissue, or insert implants either in front or behind the muscle. These incisions are then closed using dis-solvable stitches. Implants inserted behind the muscle usually look a lot more authentic and real than those in front of the muscle, therefore that procedure is also more expensive. The implants themselves are commonly silicone based and gel filled, but implants do vary in type and price and it is important that you explore all of your options and select the best one for you. Above anything, do not simply select a treatment based on cost alone, as cost-cutting could lead to disastrous outcomes. This is also key when selecting a reputable surgeon.

Length of the Operation – 1 to 2 hours.

Anesthesia – Local anesthetic with sedation, or general anesthetic.

Hospital stay – Breast enhancement is usually a ‘day case’, but when appropriate there might be 1 overnight stay.

Possible side effects – Temporary soreness and swelling change in nipple sensation. The breasts can be sensitive to stimulation for a few weeks.

Risks – Every procedure carries some uncertainty and risks. Most common problems are bleeding or infection, capsular contracture (hardening of the breast due to the formation of scar tissue around the implant); increase or decrease in sensitivity of nipples or breast skin (this can occasionally be permanent.)

Back to work – Patients can usually go back to work within a few days to 2 weeks

Fading of scars – Scars fade within several months to a year, occasionally more.

There are several options available to you which will give you different looks and flatter different people; these types of breast implants include round implants or teardrop/anatomical implants; people come in all different shapes and sizes, and that’s why it’s so important for breast implants to come in different shapes and sizes too. The most popular choice of breast implant is round implants; people who go for round implants tend to desire more lift, fullness and cleavage to the breast, however these implants can look slightly unnatural, but do have lots of positive features. Patients looking for a more ‘natural breast look’ might go for teardrop or anatomical breast implants; these implants give a full look to the breast while keeping the natural shape of the breast, however these implants need a textured surface to stop them rotating, which is not something that is a problem with the round implant. The benefits of choosing anatomical implants also means that you can customise the size and volume of your implants, whereas round implants are a more ‘one-size-fits-all’ option.

Mr Marando is a highly respected plastic surgeon who is renowned for the consistency and high satisfaction amongst his patients and procedures. During your first consultation, Mr Marando will discuss with you what you want to specifically achieve from your breast procedure. Mr Marando will show you before and after photos of what can be achieved with the different styles of breast implant and will assess your specific suitability for either round or teardrop shaped breast implants. Part of which implant you choose is what you want to achieve from your breast operation and how you want your breasts to look after the operation, so it is really important that you go to the consultation with a strong idea of what you want to achieve from your surgery to enable Mr Marando to appropriately assess which breast implant will most suitable for you.

Procedure

Breast surgery is performed under general anaesthetic and will take between one and two hours. Sometimes, this procedure will require an overnight stay in Manchester, but mostly patients are able to return home on the same day. During the procedure, Mr Marando will make an incision in your breasts in order to insert the implant. This will either be in the crease under your breast, around the nipple or towards your armpit. During your consultation, Mr Marando will assess which method of insertion will be most appropriate for you, also taking into account which method of surgery is most appropriate for your choice of breast implant. Mr Marando will then place the implant either in front of the pectoral (chest) muscle or behind the pectoral (chest) muscle. Mr Marando mainly uses the following implants: Allergan Natrelle (Inspira and Style 410) and Nagor (CoGel, GFX and Impleo).

Which breast implant type is most suitable for you will be discussed during your consultation and Mr Marando will answer any questions that you have about the procedure and your recovery period.

Recovery

Within six to eight weeks your body should have healed fully, so you will be able to see the full results of your breast surgery. Mr Marando and his team will provide you with an aftercare plan and a breast augmentation review to make sure that you are entirely happy with your choice of breast implants.

Immediately after surgery your breasts will be wrapped in gauze and a supportive bandage to increase recovery. You will likely experience some swelling and soreness, but this should be able to be controlled with over-the-counter painkillers. You will be advised to wear a supportive bra to aid healing and support your breasts for the first two weeks after surgery. You will also have a follow up appointment in seven to ten days to assess your recovery and take out any stitches that need to be removed.

Breast Augmentation surgery is usually done under general anaesthetic (the patient is asleep), but at the Star Clinic, Manchester Mr Marando offers an alternative to general anesthesia for Breast Augmentation. This alternative is called ‘Twilight anaesthesia’, which is when the patient is under local anaesthetic and IV sedation. Mr Andrea Marando is one of the few plastic surgeons who provides this as an option, yet twilight can be a great choice when considering types of anaesthesia. Being under Twilight anaesthesia rather than general anaesthesia can shorten your hospital stay, and so make your recovery significantly quicker and more relaxing. The lack of general anaesthetic can also mean that symptoms like nausea and vomiting that often accompany a general anaesthetic are not present, or significantly lessened.

Consultation

It is very important that you, as a patient, are right for surgery under Twilight anaesthesia. An extensive consultation and discussion with Mr Marando situated at either the Star Clinic, Manchester, or at his practice on Harley Street, will make sure you are right for the procedure. Mr Marando will also assess what you want from the procedure and whether this can be achieved successfully from having a breast implant procedure under Twilight. Mr Marando will go through the procedure with you entirely, to make sure that you are comfortable and feel confident with the procedure.

As it is also highly important for Mr Marando and his team that you are pleased with the results of your surgery after the operation, it is very important that you know what you want to achieve from the surgery and you relay this accurately to Mr Marando. It is very helpful if you come to the consultation with an idea of how you would like your breasts to look and what you want the outcome of the surgery to be; this will allow Mr Marando to narrow down what the best options are for you personally and to decide how to maximise the desired outcome of your surgery.

Procedure

Mr Marando will go through the procedure in detail before your surgery if you decide to go ahead with breast enlargement under Twilight. Mr Marando is a highly competent plastic surgeon so will be able to make sure that the results from your surgery are in keeping with what you desire. During the operation, Mr Marando will position the breast implant (usually a silicone gel implant) in front or underneath the pectoralis muscle (while you are comfortably sedated and anaesthetised). With Twilight sedation you will essentially be awake for the breast surgery, but Mr Marando’s skilled team of anaesthetists will obviously make sure that you are relaxed and pain-free. Some patients will drift off into a light sleep under twilight sedation, so will appear to be unconscious, while others can actually have a conversation during the operation, yet will awake with no memory of this conversation.

The use of twilight anaesthetic and sedation will reduce or completely eradicate symptoms of nausea and vomiting that often appear after a dose of general anaesthetic, making the procedure recovery more relaxed for the patient.

Recovery

If, after your consultation with Mr Marando, you feel like twilight sedation would be the best method of anaesthesia for you during your breast augmentation then this type of anaesthesia can have a significant impact on your recovery period, whereas a general anaesthetic would mean a 12 to 24 hour stay in hospital after the procedure. Undergoing the procedure under twilight sedation would mean that this stay is not necessary and you can return home to start recovering.

It is important to give your body time to recover to get the desired results from your surgery. You may experience some pain, swelling and bruising, but Mr Marando and his team will supply you with an aftercare plan and a follow up appointment to check that you are healing well and are happy with the procedure. You can also take over the counter medication to manage pain or swelling and should not return to exercise for at least a month. Take a week or so off work until you feel ready and comfortable to return, and any scarring should begin to fade after around three months.

Breast Augmentation under Twilight sedation is a great way to help speed up your recovery after surgery and may also provide a more relaxed and comfortable recovery period with less feelings of nausea or vomiting to disrupt your healing process.
